As an extension to midterm paper, you will develop a program that tackles the public health problem discussed in your earlier assignment. This program must consider the social and behavioral determinants associated with your
specific health issues.
You will pick one theory (either individual or social-based) in which to base your program and clearly explain how your program applies the theory to achieve a desired health outcome.
Your paper should include the following:
Introduction – brief refresher on public health issues introduced in midterm, and the social determinants of health that are involved.
Background on some public health programs that have been utilized in the past, and why some are more effective than others.
Discussion of theories that are relevant to desired health outcomes.
Layout and explanation of program that will tackle public health program
Integration of health program and theory of choice.
Flesh out how your program will be tied to a specific theory of behavior change, taking into account both fundamental and proximal determinants of health.
